Documents required for the Polish Embassy for a work visa

• A personal computerized passport with validity of three months after the end of your intended working contract.

• Personal identity card.
• Proof of residence if you are a national of another country.
• Formal “work permit” issued by Urząd Wojewódzki in Poland at the request of an employer.
• Any previous refusal letter of visa applications
• Formal letter of invitation from Poland. In a work visa case, it is always from the employer.
• Proof of language proficiency, educational documents, and other work experience documents, etc.
• No objection letter from the previous employer in your country of residence.
• Bank statements for the last 6 months. You will also provide your previous tax details and returns.
• Proof of ticket reservation of intended travel dates.
• You must also provide an original and copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses on the Schengen territory as well as repatriation costs for at least the first 3 months of stay or until the first payment of the health insurance tax by the employer.
